The following checklist will assist you in preparing your paperwork and registering for classes at Fulton-Montgomery Community College.

_____ FMCC Application
Fill out and return to the Admissions Office

_____ One Official High School Transcript (signed/sealed)
showing date of graduation. If you are not a high school graduate, we will need a copy of your GED AND a copy of your official high school transcript. Have your High School remit the transcript directly to the college. If your name has changed since graduation, have your High School include your current name and Social Security # on the transcript.

_____ Proof of Immunization
is required if taking 6 or more credits.

_____ Certificate of Residence
notarized at FMCC's Bursar's Office. If you are NOT a Fulton or Montgomery County resident, it must be notarized at your County Treasurer's Office.

_____ College Placement Assessment Testing & Advisement Registration

_____ Free Application for Federal Student Aid (FAFSA)
Complete and return to the Financial Aid Office. Direct  questions to the Financial Aid Office at (518) 762-4651 x7-8201. Students not possessing a high school diploma
or GED must successfully complete the Ability to Benefit (ATB) test prior to registration in order to be considered for Financial Aid. Please call the Student Development Center to schedule an appointment for ATB testing.

_____ Transfer Students need to request official academic and financial aid transcripts from any previously attended colleges. Request that both documents be sent directly to FMCC.

_____ Campus Tour or Catalog
Contact our Admissions Office at (518) 736-5300.

_____ If you have earned AP or college credits, submit an official transcript to FMCC.
